New Patient Registration

We have an open list at both our Burslem and our Norton Surgeries. In order to register with our Practice we ask for proof of address in the form of utility bill, landlords agreement etc. When you come down to surgery we will fill out a new registration form or GMS1 form and invite you for a new patient medical with a member of the nursing team. Alternatively if you register online we will contact you to arrange for a New patient Health check.

When you attend for the appointment you will be registered to the practice and your medical records will be requested from your previous GP. If you are on regular medication it is advisable to bring the right hand side of your repeat prescription to show the surgery what medication you are currently taking.

If you are unable to bring this please bring your medication down with you so that we can record everything onto our system.
